If you are using Chrome browser, you can complete the forms online by adding the PDFFiller App.  Click the button to add it.  Then Download the forms you need to your computer, open the PDFFIller App, and follow the instructions. Otherwise, just print the forms, complete them by hand, and turn them in.  Click the buttons to open them and then save them to your computer (download).
